Job Descriptions

1. Provide safe and efficient clinical pharmacy service to the area(s) assigned to meet patient needs:

• Ensure clinical appropriateness and monitoring of drug treatments, including efficacy, adverse effects, drug interactions, compatibility. 
• Provide recommendations for therapy continuation or modifications to the attending physicians. 
• Participate in patient care rounds and consult with healthcare providers to ensure most effective use of drug therapies.
• Provide medication history taking, reconciliation and counselling services.
• Coordinate efficient discharge planning (e.g. timely organisation of take home medications, discharge counseling for identified patients, etc.).
• Provide high-quality medicine information.
• Serve as a key player in multidisciplinary teams.
• Educate patients/families/caregivers as needed.
   

2. Participate in hospital formulary management process:

• Participate in evaluation and sharing of New Drug Request (NDR).
• Engage in Drug Use Evaluation (DUE) especially on new drugs introduced to the department within 6 months, to improve quality, patient and drug safety and/or cost-effectiveness
   

3. Support provision of hospital medicine information and clinical-related training to enhance staff competency:

• Participate in updating educational materials, in-house references, pharmacy bulletins and clinical information for both internal and external audiences in timely manner.
• Support the adverse drug reaction reporting system, analyse reporting trends and provide suggestions to the Pharmacy and Therapeutics
• Committee (P&T) for improvement initiatives. 
  Provide education and training to healthcare professionals, including pharmacy staff, nurses and allied health professionals

Qualifications & Experience

• Recognized Degree in Pharmacy by the Malaysian Pharmacy Board.
• Candidate with clinical pharmacy experience is prioritized.
• Minimum 2 years' experience in hospital pharmacy setting 
• Current registration as a practicing pharmacist with the Malaysian Pharmacy Board

Job Descriptions

1. Ensure all prescriptions are accurately dispensed to the correct patient at all time

• Check all medications supplied are according to the prescription with accurate labels and correct physical medications
• Review appropriateness of prescription and verify with prescriber if necessary. Seek advice from pharmacist where required
• Counsel patient on proper medication administration & technique (where applicable) and advise on appropriate storage condition
• Provide alternatives or suggestions on medication treatment where appropriate
• Provide basic medicine information e.g. indication, dosage, adverse effects, method of administration, etc.

2. Ensure medications are stored according to recommended conditions to maintain efficacy and quality

• Adhere to recommended storage requirements when handling medication in the pharmacy at all times

3. Accurate documentation to ensure compliance to regulatory requirements and accreditation standards

• Document all daily transactions completely and accurately in the required manner e.g. Poison Register, Dangerous Drug Register, Psychotropic Drug Register, Codeine & Pseudoephedrine Register, in accordance to regulatory requirements and accreditation standards.

4. Ensure quality and sterility of medication

• Adhere and prepare formulation in accordance to Good Manufacturing Practice e.g. wearing of Personal Protective Equipment during manufacturing.
• Perform cytotoxic drug reconstitution using aseptic technique in accordance to procedure.

5. Ensure all ward stocks are accurately supplied to wards and departments

• Prepare and supply all ward stocks according to requirements.

6. Ensure standards & customers' expectations are met

7.Perform any other duties assigned to meet company and department goals and objectives

Qualifications & Experience

• Diploma in Pharmacy from a recognized institution, or certificate of dispensing/relevant certificate from a recognized institution
• Experience in hospital pharmacy is an advantage. Fresh graduates may also be considered

CORE DUTIES AND RESPONSIBILIES

1. Clinical Function:

• Competent and efficient in conducting laboratory testing that provides crucial information for detecting, diagnosing, treating, and monitoring diseases.
• Adhere to and execute tasks according to the hospital’s and laboratory’s policies and procedures. 

2. Administrative Function: 

• Ensure that inventory control is properly implemented, i.e. to ensure items received do not have short expiry dates, notify immediate superiors on shortage of supplies, monitor stock inventory record.
• Perform quality control, institute preventive maintenance of equipment, and ensure policies and procedures are followed so that test performances are at an acceptable level and standard in accordance to Laboratory MS ISO 15189 Quality Manual. 
• Perform and monitor daily quality control (QC) of the tests, institute corrective and preventive maintenance of equipment and ensure policies and procedures are followed 
• Perform or carry out laboratory testing and tasks according to laboratory’s policies and procedures
   

QUALIFICATIONS & REQUIREMENS  

• Bachelor of Science or; Bachelor of Biomedical Science, recognized by the Government of Malaysia.
• Basic Good Laboratory Practices (GLP).
• Excellent communications skills, compassionate.
• Effective decision-making and problem-solving skills.
• Demonstrate high initiative and work independently towards achieving goals and objectives competently
